At 1st Care Management, our nurse practitioners and physicians travel to patients throughout Metro
Atlanta — conducting comprehensive assessments, managing medications, ordering and reviewing lab
work, and coordinating with specialists — all without requiring the patient to leave home.

After receiving a referral or new patient request, our team contacts you to schedule a visit at a time that works for you and your family. We accommodate caregiver schedules so that family members can be present during the appointment.
During the visit, your nurse practitioner conducts a thorough evaluation — including vital signs, a review of current symptoms, a full medication reconciliation, and a physical assessment tailored to your conditions. Because the visit takes place in your home, the clinician can also evaluate environmental factors that impact your health and safety, such as fall risks, accessibility issues, and medication storage.
Following the assessment, your clinician updates or creates a care plan, coordinates any necessary referrals or lab orders, and schedules your next visit. You receive clear instructions and have the
opportunity to ask questions in a relaxed, unhurried setting — something that is rarely possible in a busy clinic.
