1st Care Management was founded in 2014 with a straightforward conviction: the highest quality healthcare should not require a trip to the doctor’s office. For more than a decade, we have served seniors and homebound patients throughout the Metro Atlanta area — delivering clinical care directly to the homes of the people who need it most.
Our practice is headquartered in Sandy Springs, Georgia, and our clinical team travels across Metro Atlanta — including Roswell, Dunwoody, Marietta, Decatur, Alpharetta, and surrounding communities. We are not a staffing agency and we are not a home health aide service. We are a licensed medical practice that provides physician-supervised primary care, transitional care management, and chronic care management through in-home visits.
We understand that circumstances can make going to the doctor’s office an incredibly stressful and taxing event. We strongly believe that access to healthcare should never be dictated by an individual’s ability to leave the home. Through our combination of services, we have built a proven track record of decreasing hospitalization rates and reducing overall healthcare costs for the patients and communities we serve.

We believe that a patient’s ability to access quality healthcare should never depend on whether they can get to a doctor’s office. For many seniors across Metro Atlanta, a routine appointment means arranging transportation, navigating a parking garage, sitting in a crowded waiting room, and enduring the physical toll of a trip that could have been avoided entirely. For patients recovering from surgery, managing chronic illness, or living with mobility limitations, that burden is not just inconvenient — it can be dangerous.
