About This Role

1st Care Management is seeking board-certified or board-eligible physicians in Family Medicine or
Internal Medicine to join our Sandy Springs office. This is an in-office clinical role supporting a practice that delivers transitional care management, chronic care management, and in-home medical visits to elderly and homebound patients throughout Metro Atlanta.

As an in-office physician, you will serve as a clinical anchor for the practice — diagnosing and treating patients, supervising Nurse Practitioners conducting field visits, reviewing care plans, preparing clinical reports, and ensuring that every patient receives timely, accurate, and compassionate care. You will work closely with our care coordination team to manage a patient population that depends on consistent, high-quality medical oversight.

This is not a high-volume, revolving-door clinic. Our patients are seniors with complex conditions who benefit from the kind of thoughtful, relationship-based care that is increasingly rare in modern medicine.

Responsibilities

  • Provide direct patient care including examinations, diagnosis, treatment planning, and follow-up for patients seen in the office and referred from the field
  • Supervise and collaborate with Nurse Practitioners who conduct in-home patient visits, reviewing their assessments, care plans, and clinical documentation
  • Diagnose common and complex illnesses, providing specialist referrals when appropriate
  • Educate patients on disease prevention, health maintenance, and self-management strategies, ensuring clear and detailed follow-up communication
  • Prepare and maintain accurate medical records and clinical reports in the EMR system
  • Administer or order examinations, vaccinations, and diagnostic testing as clinically indicated
  • Participate in care coordination for Transitional Care Management (TCM) and Chronic Care Management (CCM) patients, ensuring continuity across the hospital-to-home transition
  • Collaborate with the administrative team on quality improvement initiatives, clinical protocols, and patient outcome tracking
  • Maintain board certification and stay current with continuing medical education requirements
