About This Role

1st Care Management is seeking a board-certified physician in Family Medicine or Internal Medicine toserve as a Supervisory Physician for our team of field-based Nurse Practitioners. This is a primarily
remote, work-from-home position with on-call responsibilities and periodic in-office requirements.

In this role, you will provide the clinical oversight that allows our NPs to deliver high-quality in-home medical care to elderly and homebound patients across Metro Atlanta. You will review patient
assessments, collaborate on care plans, provide guidance on complex clinical cases, and ensure that every patient receives physician-supervised care that meets the highest standards.

This position is designed for a physician who wants to maintain active clinical involvement and
leadership without the demands of a full-time office or hospital schedule. You must live in or near the Sandy Springs, Georgia area to be available for in-office collaboration when needed.

Responsibilities

  • Serve as the supervising and collaborating physician for Nurse Practitioners conducting in-home patient visits across Metro Atlanta
  • Review NP clinical assessments, care plans, and documentation to ensure accuracy, completeness, and clinical appropriateness
  • Provide real-time clinical guidance and consultation to NPs in the field via phone, secure messaging, or video when questions or complex cases arise
  • Co-sign orders, prescriptions, and clinical documentation as required by Georgia supervisory and collaborative practice regulations
  • Participate in care coordination for Transitional Care Management (TCM) and Chronic Care Management (CCM) patients, ensuring continuity and quality across the care continuum
  • Review and approve referrals to specialists, diagnostic labs, and community health resources
  • Assist with credentialing, quality assurance, and clinical protocol development as the practice grows
  • Attend periodic in-office meetings, case reviews, and administrative sessions as required (frequency to be determined with leadership)
  • Maintain board certification and stay current with continuing medical education requirements
  • Be available on-call to support NPs during scheduled home visit hours
